To be better prepared for your examination, we ask that you download, print and fill out our Head, Neck and Facial Pain Questionnaire and TMJ Scale. Please take the time to complete the forms and bring them with you to your appointment. This information, along with the examination, will help us make an accurate diagnosis. Please list any/all doctors you have seen for this condition and all medications (including vitamins), that you take on a regular basis. And please bring any night guards or splints you might be wearing.
