To be better prepared for your examination, we ask that you download, print and fill out our Head, Neck and Facial Pain Questionnaire and TMJ Scale, included with our New Patient Package (below). Please take the time to complete the forms and bring them with you to your appointment. This information, along with the examination, will help us in making an accurate diagnosis. Please list any and all doctors you have seen for this condition and all medications that you take on a regular basis. And please bring any night guards and/or oral splints.
